I have to say, this development is thoroughly shocking to me. The IPR ordinance says that once they receive your notice to appeal ("review") they schedule the hearing before CRC. They are using their administrative authority to screen out cases before CRC even knows about them.
I know you are inclined to tell your whole story, but here's what I suggest:
You should send an email to the CRC including just the letter from Director Severe saying:
Dear members of the Citizen Review Commitee:
I filed a timely appeal of a complaint about misconduct which resulted in an investigation and findings. I received this letter from IPR asking for more information and stating they were going to schedule a hearing. Shortly after that, they cancelled the hearing because I did not respond right away.
Per City Code 3.21.150

when the Director receives and accepts a timely request for review, A Case File Review and Appeal Haering shall be scheduled before the Committee.
